Denial of Disability Insurance Claim

Summary: This complaint states as follows: Dr. Josh Brower, a dentist and former U.S. Army Captain, had long-term disability insurance with Great-West Life & Annuity Insurance Company (now Empower Annuity Insurance Company of America).
